[HTTPS-Everywhere] No rules, can't update, can't remove

Unfortunately Tony didn't leave a reply address :(.

These UI symptoms indicate that some part of the JS / XUL / CSS code hit a
fatal exception.  It would be most useful to look at the Firefox error console
and see what that exception was.
