[HTTPS-Everywhere] Ruleset for Dutch general practitioners

Hi,

I've attached a ruleset for Praktijkinfo, a website engine used by 1.759
Dutch general practitioners. By default, it switches to SSL for private
information, but this ruleset forces it to use SSL for all pages.
