[HTTPS-E Rulesets] New rulesets (iCarol, Lifeline Australia, ) plus other extended/fixed rulesets

At the git://github.com/galenide/https-everywhere.git repository, new
rulesets for iCarol and Lifeline Australia are available, along with
a partial ruleset for OpenX. In addition, there are extended rulesets
for Give Kids the World, Instagram, and for the US Dept of Veterans
Affairs, along with an improved Harland-Clarke ruleset.
