[HTTPS-E Rulesets] Rules for German health insurance websites.

Hello.

attached you'll find rulesets for German health insurance websites. In
Germany you have to be health insured, but you can choose a vendor.

There may be some missing due to the fact that "BKK" got many different
sites. IKK got many too, but I checked all those. They do not seem to
support https, but I included those which do.

Also the private insurance companies are missing.
